When entering your distances, the format is Carry-Roll-Total, so your entry for a 9″ Backswing might be 03-08-011. Yes, you need to enter a leading 0 if your distance is less than 10 for Carry and Rollout, or less than 100 for the Total.
You are free to use feet or yards, though it makes more sense to use feet when so close to the green. If you are more than 200 feet from the pin you probably should be looking at your Distance Card rather than your Chip Card.
